Social Services

A bilingual social worker at CVIM provides individuals and families with referral services, case management, and counseling in both English and Spanish. Applications for state benefits, health and human services programs can be submitted on behalf of patients. In collaboration with the medical and dental team, the social worker assures patients’ basic needs are being met to facilitate healthy living and/or appropriate management of chronic illness.

Since 2011, CVIM has partnered with West Chester University, serving as a field placement for social work interns pursuing their bachelors or masters of social work.  CVIM strives to provide holistic care and promote well-being.

Referrals may include but are not limited to:

  • State Benefits (food stamps, child care subsidy, medical assistance, etc.)
