|Our phone lines are staffed primarily by volunteers. Calls are usually returned within a day between 9:00 am and 3:00 pm Monday through Friday. If you have to leave a message, please make sure you give a number where you can be reached during those hours.
The volunteer eligibility screener will ask questions about your household and your household finances. You will be asked to bring household income documentation to your eligibility screening interview.

|Step 2: Collect Your Documents (New and Rescreen Patients)|
|You must provide the required document (to the right) for a completed screening. Any missing information will place your application in a pending status until all documents are received and reviewed.||REQUIRED DOCUMENTS for ELIGIBILITY SCREENING:|
|Step 3: Come to CVIM During Screening Hours (New and Rescreen Patients)|
|Bring your required documents with you!
New patients must schedule a registration interview. See Step 1 for details.
If you DO qualify for medical and/or dental services, an appointment will be made for you at a later date.
If you DO NOT qualify for services at CVIM, we will try to refer you to other community agencies which may be able to assist you.
