See the list of required documents below to start the renewal process, once received documents will be reviewed and we will call you for verification. If documents are missing, your application will be pending until we receive all the documents needed.

*Sending your documents does not automatically make you eligible for CVIM services. It will take 6-8 weeks to process.

Once you are eligible:

• YOUR REGISTRATION EXPIRES EVERY 12 MONTHS OR IN CASE OF A LIFE CHANGING EVENT
• YOU CAN REAPPLY UP TO 3 MONTHS PRIOR TO DATE OF EXPIRATION OF BENEFITS
• YOU MUST NOTIFY CVIM IF YOU ARE GETTING MEDICAL OR DENTAL INSURANCE

PACIENTES ESTABLECIDOS:

Vea la lista de los documentos requeridos para empezar el proceso de renovación, una vez recibidos se revisarán y le llamaran para verificar los datos, si falta algún documento su aplicación quedará pendiente hasta que hayamos recibido todos los documentos requeridos.

*Él envió de documentación no los hace elegibles automáticamente para servicios en CVIM. Tarda entre 6-8 semanas para procesar los documentos.

Una vez elegible:

• SU REGISTRO VENCE CADA 12 MESES o SI HAY ALGUN CAMBIO EN SU SITUACION DE VIDA
• EN CASO DE OBTENER SEGURO MEDICO O DENTAL DEBE INFORMAR DE INMEDIATO A CVIM
• PUEDE RENOVAR HASTA CON 3 MESES DE ANTICIPACIÓN ANTES DE QUE SE VENZA SU APLICACIÓN
